It won't boot from HD unless you do steps 4 and 5 from first post. The OS X installer doesn't write to boot sectors, it shouldn't interfere in Windows boot before you do step 4.

Looks like a filesystem problem, try booting with -s and run fsck -fy It may be a harddisk problem https://discussions.apple.com/thread/1810401

No idea. You should be able to boot again typing GraphicsEnabler=No at boot. What about those ATY_init messages? Did you install this kext? If you did, you should remove it before using GraphicsEnabler.

Use boot verbose (-v). Don't need to specify PciRoot, zero is default value.

No, you got "_UID" = "0", not 1. Did it work with PciRoot=1?

Faltou incluir CHIP_FAMILY_SEYMOUR em chip_family_t, observe no código de onde você copiou essa linha. Também deve incluir "Seymour" em chip_family_name[], não vai dar erro na compilação se não incluir, mas talvez problema na execução.

The original DSDT sometimes isn't good for OS X because it's designed to run on Macs and not on ordinary PCs. You don't need to extract the DSDT if you won't patch it, the bootloader gets it from BIOS. You know if patched DSDT is good by testing. -

There was 2 identical patches for different mobos with BIOS 2.20, so I replaced by a generic one, it should work for any ASRock with new BIOS. -
Older version of IASL. The editor doesn't matter, can be any. If you use DSDT Editor, replace iasl in Contents/Resources/Java. I don't know about multishit, but the rest is correct. I don't get why someone needs to use a tool to something as simple as drag and drop a file to a folder. It won't be patched unless you patch it first.

You are missing for VoodooHDA Not related to PCIRootUID. VerboseLevel means how much info it shows in log.
