You have to look for information in the KP screen. Look for "kernel extensions in backtrace (with dependencies)". In SMBIOS you should use MacPro4,1 I believe.

Use the verbose boot (-v) so when the KP happens you should see some (maybe) useful information on the screen, instead of the "turn off your computer" message.

Tente remover as kexts de vídeo do pen drive e use no modo VESA, não há necessidade de ter o vídeo acelerado durante a instalação. Depois, com o sistema instalado, você ativa o vídeo conforme o tópico que o Artur indicou.

Você deve ter editado alguma coisa errada, só precisa adicionar as duas linhas em vermelho. Porém outro membro com o mesmo problema já testou e reportou (na seção em inglês) que não adiantou.

Pelos posts acima entendo que ele usou DVD retail e o HackBoot mais recente.
