same thing happened to me, what i did was reset my bios, adjust vt-d and visualization setting in bios (on/off depending upon your system) and it got fixed

Fixed sleep wake issue by using iMac15,1 SMBIOS

is there is any way we can make sure that the graphics card is troubling sleep? like logs or dump?

Sleep works when i try to wake it after 2 minutes it work fine but after a litter longer time like 5 minutes it wake but no display and later it restart
